Managing a rheumatology condition can be challenging. Chronic Care Management helps eligible patients receive ongoing support outside of and between doctor visits, so you do not have to manage your health alone.
Chronic Care Management, often called CCM, is a service for patients living with two or more chronic conditions expected to last at least 12 months or longer. It is designed to improve continuity of care outside the exam room.
For Denver Arthritis Clinic patients, CCM can be especially helpful when managing ongoing rheumatologic and related medical conditions that require regular monitoring, medication management, follow-up communication, and coordinated care.
It adds another layer of support between appointments by helping patients stay connected to their care team, their treatment plan, and their health goals.

Patients must have two or more chronic conditions expected to last at least 12 months or for the rest of the patient’s life.
Your provider discusses the benefits of CCM, why you may be a good candidate, documents the discussion, and may prescribe CCM services.
Enrollment in CCM requires your consent before the service begins.
A comprehensive electronic care plan is created to include your goals, interventions, ongoing monitoring, and follow-up needs.
