What Is Chronic Care Management (CCM)?

What to expect with CCM services

Eligibility

Patients must have two or more chronic conditions expected to last at least 12 months or for the rest of the patient’s life.

Initial Visit

Your provider discusses the benefits of CCM, why you may be a good candidate, documents the discussion, and may prescribe CCM services.

Patient Consent

Enrollment in CCM requires your consent before the service begins.

Care Plan

A comprehensive electronic care plan is created to include your goals, interventions, ongoing monitoring, and follow-up needs.

Care Coordination

This may include communication with patients and healthcare professionals, medication adherence support, referrals, and assistance with scheduling.

Supervision

Services are provided under provider supervision, and only one provider may bill for CCM services during a calendar month.

What does CCM cost?
